My District office staff will make every effort to assist you to obtain timely and accurate information from federal agencies.
The first step is for you to submit a signed copy of a Privacy Act Release – Congressional Inquiry and any supporting documentation to either of my district offices (addresses and fax numbers provided below). This release provides my staff with your permission to discuss the case with the appropriate federal agency. In addition, a letter explaining your request may be helpful in gaining a better understanding of your issue. Supporting documentation provides relevant background information and a more complete picture of your situation.
Once my staff receives your signed Privacy Act Release – Congressional Inquiry and other documents, we will take the appropriate action on your behalf. A staff member will contact you if more information is needed or to inform you of the steps that have been taken on your behalf.
If you need assistance with an immigration issue, please provide a signed copy of the Privacy Act Release – Congressional Inquiry (Immigration) instead of the overall Privacy Act release.
